ESP全自动快速血培养系统临床应用评价

[摘要] 目的 评价ESP全自动快速血培养系统的功能及影响因素。 方法 应用ESP检测临床标本416份(普通血培养388份,痰结核培养28份),观察其阳性检出率、检出时间,并对所检出的病原菌进行分析。观察不同细菌浓度、温度和放置时间,以及抗生素对检测结果的影响。 结果 388份血培养中检测出阳性标本68株,阳性率为17.3%,假阳性为1.3%,其中24小时内检出67.6%,48小时内检出25%,48小时后检出7.4%。分离出病原菌23个种属。检测速度与细菌浓度成正比;一定量的菌液浓度加入培养瓶内,放置机外不同湿度、不同时间对阳性检出率影响不大,但总的报告时间延长;培养瓶可以中和一定浓度的抗生素。28份抗酸染色阳性的痰液标本,ESP检出结核杆菌25份,与常规培养一致,最快检测时间4.74天,最慢30.93天,医疗器械平均为15.9天。结论 ESP仪是一种具有多功能的自动化程度高、快速、正确等优点的培养系统。

[Abstract] Objective To evaluate Difco ESP blood culture system and analyze the positive rate effected by different factors. Methods 416 clincal specimens (blood specimen 388 sets, suspicious TB sputum specimens 28 sets) were incubated by ESP. The positive rate, speed, false positive rate and the influent factors were observed and analyzed. Results In a total of 388 blood culture sets, 68 cases were positive (17.3%). Among them, 46 cases (67.6%) positive in less than 24 hours, 17 cases (25%) positive during 24 (48 hours), and 5 cases (7.4%) positive over 48 hours. The incubate speed had good relationship with bacterial concentration in the specimens. The positive rate was not effected when the culture bottles hold in different time and temperature in vitro. And the antibiotics in the specimens could be neutralized by the culture bottle. In the suspicious TB sputum specimens, 25 cases were positive, as the same with the routine culture result. The fastest incubation time of TB was 4.74 days, and the slowest time was 30.93 days. Conclusion Difco ESP blood culture system was a speedy, reliable, and multi-function culture system.

近年来,自动化非介人性连续瓶外检测的血培养仪已逐渐在国内推广使用,主要机型有Difco公司ESP系统、BD公司的Bactec9000系统和生物梅里埃公司的VITAL系统等,对提高血培养的阳性率,快速准确地作出病原学诊断提供了十分有利的条件。我们引进美国Difco公司研制的最新产品ESP全自动多功能培养系统已近一年,现就临床应用方面的评价报告如下。
